Medication Form
If your child needs to take medication during the school day, we cannot administer this unless we have a completed medication form.
You can pop into the office to complete one or download and print the one below at home yourself and bring it in OR email it to us at office@woodlandview.notts.sch.uk.
You must bring the medication in for your child and we can only hand it to an adult to remove from school grounds. Children cannot take their own medicine home.
